Please read and fill out all forms correctly. Click on the student form that is needed and print out the form. Bring the form to the main office. Please remember to have a parent/guardian signature on the forms that require this information.

• Student Emergency Form
  • Please notify the main office of any changes on this form throughout the school year.
• Student Medication Form
  • Medication needs to be in the original labeled container and a prescription for an inhaler must obtain an additional release form from the school nurse.
• Student Education Visit Form
  • Requests should be submitted to the Principal ten (10) days prior to the first day of the trip.
• Work Permit Application
  • Parents should fill this application out and take to either the high school or middle school office, with a copy of the student's birth certificate for issuance of the work permit.
